Emergency Cards
Emergency cards were done on line this year.

All information should have been completed on line by parent/guardians, by Sept 23, 2019.
If you did not do this, you will need to fill out a paper form and return it to the front office.  These forms may be obtained in the front office. 

Please note that no medications can be given to your student until this form is completed.

It is important that any changes in contact information be communicated to the nurse or main office, so the school is able to contact the parent/guardian in the event an emergency occurs.

Please make sure to list at least one emergency contact,
in case that we are not able to contact you.
